Darebin Community Health

  • Monthly Paediatric and Vitamin D Clinic, for children of Refugee background to 18 years

For paediatric assessment and management (secondary referral not screening) from a visiting paediatrician from RCH.
Clinic has access to RCH large dose Vitamin D formulation for doses that last for 6 months – 1 year, making it less expensive and easier for families.
Available for children from refugee background across the Northern area.
At Darebin Community Health, 125 Blake St, Reservoir.

Plenty Valley Community Health

Healthy Mothers, Healthy Babies Program - Information for GPs

The Healthy Mothers, Healthy Babies (HMHB) Program provides:

  • assertive outreach to pregnant women who may be difficult to engage, or have complex needs,
  • community based support to women in the target group up until 4-6 weeks post delivery,
  • support to link the woman with clinical antenatal care providers and maternal and child health nurses,
  • assistance to link with a range of health and human service providers as required,
  • health information/ education especially about the impacts of drug and alcohol use, smoking, healthy eating and exercise, and breastfeeding,
  • opportunities for peer support from other pregnant women.

HMHB Program can provide support to GPs who have concerns that their patients may be falling through the gaps of the system or require more support than the usual system can provide.
The program is delivered local government areas in metro Melbourne that have high numbers of births, higher rates of relative socio-economic disadvantage and lower service availability - this includes City of Whittlesea. The agency delivering the program is Plenty Valley Community Health.
