Immunisation History forms
Immunisation history forms (A4 purple forms) are only required to be filled out if there is an error with the child’s immunisation history. Doctors are not required to fill out the whole history of the child, only the section that is missing. If ACIR has not been notified of the child’s immunisation and you are informing them for the first time, then you are entitled to a notification payment of $6 even though you haven’t administered the injection.
