Hospital Inreach & Residential Response Services for Residential Aged Care
What is Residential Response Service?
Frail, elderly residents of Nursing Homes and Hostels often suffer significant complications such as delirium, falls and pressure areas when hospitalised. The Residential Response Service provides an alternative model of care for those RACF patients whose care needs can be safely met in their own environment.
The service is available 7 days a week. The service includes:
- Geriatricians, Hospital Medical Officer, Nursing & Social Work.
- Access to Allied Health services such as Physiotherapy, Occupational Therapy, and Speech Pathology as required.
