The Division is involved in many local health promotion initiatives in addition to running a health promotion program of its own. The Divisions health promotion activities are wide and varied; in fact the Division endeavours to incorporate health promotion into all programs and activities, recognising the increasing importance of framing health within a population based approach aimed at preventing ill-health.
What is population health?
According to the Joint Advisory Group on General Practice population health is:
"The prevention of illness, injury and disability, reduction in the burden of disease and rehabilitation of those with a chronic disease. This recognises the social,cultural and political determinants of health. This is achieved through the organised and systematic responses to improve, protect and restore the health of populations and individuals. This includes both opportunistic and planned interventions in the general practice setting. "
SNAP Framework
The Joint Advisory Group (JAG) developed the SNAP framework for population health in General Practice. The framework targets four behaviours that are known to impact on chronic disease and other health issues – Smoking, Nutrition, Alcohol consumption and Physical activity. The Division has adopted the SNAP framework for the health promotion program, not only does the framework provide a structure to organise Division health promotion activities, but it has provided a common language to communicate with other health promoting agencies in the northern metropolitan region of Melbourne.
Lifescripts
Lifescripts supports general practices in managing five major risk factors: smoking, nutrition, alcohol, physical activity and weight management. Lifescripts provides tools and information for GPs, their staff and and patients to tackle these critical lifestyle areas. The Northern Division supports local GPs implementing Lifescripts.
