HARP Program


 

What is HARP?

HARP is the Hospital Admissions Risk Program.
It is part of the Hospital Demand Management Strategy of the Victorian Department of Human Services (DHS).

HARP aims

The overall purpose of HARP is to reduce the avoidable use of hospitals through preventive models of care, focusing on people with chronic and complex health care needs and giving priority to high volume or frequent users of the acute public hospital system.

Many HARP programs begun in 2003 are now being integrated into mainstream services.
