GPs in Darebin and Whittlesea now have a single point of contact to coordinate the assessment, treatment and management of their diabetic patients.
- Do you need assistance with the ongoing care of your patients who have Type 2 Diabetes?
- Do those patients live in Darebin or Whittlesea?
Diabetes Direct is accepting referrals to provide an integrated multidisciplinary and holistic approach to diabetes management. The project targets people who present to The Northern Hospital’s emergency department or have unplanned hospital admissions due to unstable diabetes.
The free services offered are:
Endocrinology Mon am and Wed pm
Dietetics Mon-Wed
Podiatry Tues-Thurs
Diabetes Educator Tues-Thurs
Physiotherapy Mon & Thurs
Referral guidelines and assistance
To ensure an efficient and effective service, recent pathology results and the completed Victorian Statewide Referral Form (VSRF) should be faxed to 9408 9508. The VSRF can be found in Medical Director (Supplied Template from Letter Writer) or from GPV's website. Click here.
