The Respecting Patient Choices program at Northern Health aims to facilitate discussions with patients, and to assist in the development and implementation of advance care plans through education and support to patients, their families and carers.
Education and assistance is also provided to health professionals, including hospital and community doctors, nursing and allied health staff.
This process helps families, treating doctors and other health care professionals increase their awareness of a patient’s health care priorities in the event that this person is unable to make decisions on their own behalf.
The Respecting Patient Choices program is funded by the Victorian Department of Human Services. It involves collaboration between Austin Health and Northern, Eastern and St. Vincent’s Health, residential aged care facilities and GPs. It is based on a model developed in Wisconsin, USA.
